Use the Additions Application Form to apply for approval to install, replace or alter decks, patios, sidewalks, driveways, hot tubs, sport courts, exterior heat pumps and a/c units, swimming pools, antennas, satellite dishes, awnings, propane tanks, dog runs, trellises, arbors, gazebos, landscaping, building additions, and other projects that would change the exterior of your property and/or home. The list of approved decking materials can be downloaded below.

In addition to ACC approval, you may need a permit from the City of Mill Creek as well. Be sure to read the City letter available below.

You may download the ACC form in either PDF or MS Word format below. PDF format is usable from both PC's and Mac's, but requires that you have the free Adobe Acrobat Reader installed on your computer (available here). To use the PDF form, you can save it on your computer or just print it directly from the screen. PDF forms must be filled in by hand.

You may also download the form in Microsoft Word format and fill it in directly on the screen using your computer, or save it and fill it in later. This requires that you have MS Word on your computer (Version '97 or later).
