Submit the Tree Trimming / Removal Application Form to the MCCA Office to request approval for tree trimming or removal.  Minor tree trimming is allowed without approval, but trimming that may significantly affect the appearance or health of the tree requires approval. For specifics on tree cutting rules, download and read the instructions below. Additional information is provided on the Form and in the ACC Guidelines.

In addition to ACC approval, you may need a permit from the City of Mill Creek as well. Be sure to read the City Permit Requirements available below.

You may download the ACC form in either PDF or MS Word format below. PDF format is usable from both PC's and Mac's, but requires that you have the free Adobe Acrobat Reader installed on your computer (available here). To use the PDF form, you can save it on your computer or just print it directly from the screen. PDF forms must be filled in by hand.

You may also download the form in Microsoft Word format and fill it in directly on the screen using your computer, or save it and fill it in later. This requires that you have MS Word on your computer (Version '97 or later).
