Turn off Village Green Drive onto Country Club Drive and take the second left into the parking lot on the left. The office is open 8 AM to 4 PM Monday through Friday.

ACC Roofing Application Form

Use the Roofing Application Form to apply for approval to replace your roof. Be sure to review the MCCA-approved list of roofing materials available below. Roof replacement is very expensive, so choose a contractor carefully. Roofing like Presidential TL must be carefully aligned during installation to prevent a ragged appearance. Other materials have similar requirements so look for a contractor experienced in installing the type of roofing you choose. The list of contractors previously recommended by other MCCA members is available here.

In addition to ACC approval, you will most likely need a permit from the City of Mill Creek as well. Be sure to read the City letter available below.

You may download the ACC form in either PDF or MS Word format below. PDF format is usable from both PCs and Macs, but requires that you have the free Adobe Acrobat Reader installed on your computer. To use the PDF form, you can save it on your computer or just print it directly from the screen. PDF forms must be filled in by hand.

You may also download the form in Microsoft Word format and fill it in directly on the screen using your computer, or save it and fill it in later. This requires that you have MS Word on your computer (Version ’97 or later).

Mail theft within MCCA has risen sharply since 2000!

The standard mailboxes are provided by MCCA at no cost to the residents in those divisions still using individual mailboxes. The locking mailbox is available for purchase through MCCA by any MCCA resident desiring one. It’s U.S. Postal Service approved, is constructed of heavy-duty metal, and has a larger, locked incoming mail compartment. The owner is given the only two keys to the locking mailbox when it’s installed (USPS will not have a key). Outgoing mail can be placed in an unlocked portion of the mailbox, but it’s not recommended.
